Understanding a FACIS Level 3 Background Check
Patient safety increases when health care organizations thoroughly verify the identity, credentials, and backgrounds of their providers. The level of accuracy with which these screenings are conducted matters.
Since there is not a consolidated list of bad actors and fraudulent individuals who are skilled at hiding their identities, having many data sources is critical to establishing a complete picture of someone’s history. It not only prevents potential abuse from slipping through the cracks, but it also protects the reputation and good names of providers who might be misidentified through less rigorous searches.
FACIS® (Fraud Abuse Control Information System) is the gold standard data platform and a nationally used database. It is owned and maintained by Verisys and consists of primary source content from federal and state sources for exclusions, sanctions, debarments, and disciplinary actions against health care professionals and businesses for all published license types and publishing jurisdictions.
FACIS is the most comprehensive data set for screening and monitoring health care providers and offers two different levels of searches, depending on our clients’ needs. The following are the levels available for FACIS searches, as well as what’s included in those searches and the data they produce.
What is a FACIS Level 3 background check? What does it entail?
A Level 3 FACIS background check is the gold standard for exclusion screening, verification, and monitoring. This search includes the entire FACIS database with over 5,000 data sources including historical and state data, millions of individual records, and continuous updates.
A FACIS Level 1 background screening includes the minimum standards outlined in the Office of Inspector General Compliance Program Guidance. This search includes federal databases such as the Systems for Award Management (SAM) maintained by the General Service Administration, and the List of Excluded Individuals and Entities (LEIE) maintained by the Office of the Inspector General. A Level 1 search also includes state-specific databases when they are applicable and available.
Level 3 background checks include everything found in a Level 1 screening as well as thousands of other records and databases. Over 75,000 records are added monthly, and FACIS also includes historical data dating back to 1992 and continues to monitor providers after their initial screening. FACIS searches can be delivered via the web platform or through a SFTP or other integration.

Since many health care organizations receive funding from federal sources, a search of federal exclusion lists is vital. Payments to individuals on the exclusion list are costly, both in legal fees and fines, and there is a risk to patients and a potential loss of reputation.
These databases receive regular updates, so keeping up on those additions is an important part of continuous monitoring—FACIS pulls that data as it is published. Verisys’ platform matches and compiles the data, cross checking for errors and irregularities, and it provides a record that is easy to read and use.
Other databases included in a FACIS Level 3 search
A FACIS search looks for results or potential matches from all federal data sources including:
- OIG (OIG database verification)
- SAM (including SDN)
- FDA Disbarment
- U.S. DOJ
- U.S. Treasury Dept
- U.S. State Dept
A FACIS Level 3 search also includes all state-level Federal Healthcare Entitlement Program (FHEP) sources such as: State Medicaid Exclusions, State Contractor Disqualification/Debarment Lists, HEAT Task Force News, State Attorney General News, Federal District – Attorney General News, and Medicare/Medicaid Opt-Out Lists.
