CheckMedic® Privacy Notice

This Privacy Notice applies to the collection of personal information by Verisys Corporation when using or accessing the CheckMedic application and related services.

What is CheckMedic:

CheckMedic, by Verisys, is a credentialing solution for healthcare providers and organizations to systematically manage and primary source verify professional background, licensing, and credentialing information.

CheckMedic collects a provider’s personal and professional information on behalf of our customers (healthcare organizations and employers). The system creates a digital credentialing profile, called a MedPass®, for each provider, verifying their information using primary sources. CheckMedic can also collect other critical data (e.g. immunization records) on the organization’s behalf and monitor that data in real time so that organizations can uphold their credentialing and screening obligations.

Information We Collect

We collect personal information in accordance with applicable law and for the purposes set forth in our Consumer Data Privacy Policy. This information may be collected from the healthcare organization or directly from the consumer in cases where consumers access and manage their own MedPass.

Verisys may collect the following personal information for your CheckMedic MedPass:

Personal Identifying Information	This includes your name, address, telephone number, and social security number, date of birth, email address.
Characteristics of Protected Classifications	This includes any items that may identify your membership in a protected class such as: Sex/gender (includes pregnancy, childbirth, breastfeeding and/ or related medical conditions) Marital status Medical Condition (genetic characteristics, cancer or a record or history of cancer) Military or veteran status National origin (includes language use and possession of a driver’s license issued to persons unable to provide their presence in the United State is authorized under federal law) Ancestry Disability (mental and physical including HIV/AIDS, cancer, and genetic characteristics) Genetic information Age (over 40)
User Identification	This includes information that is necessary to log into and manage your CheckMedic account, such as: Username Unique Identification number User email address
Device Identifiers	This includes the identifiers that are unique to the device you use to access CheckMedic. Including: Device type or name Apple’s identifier for advertisers (IDFA) Google Advertising ID (GAID). International Mobile Equipment Identity (IMEI) Media Access Control (MAC) address Secure ID Internet Protocol Address (IP address).
Professional and Employment-Related Information	This includes any records related to your work history and ability to work in your licensed profession. Professional license(s) Professional certification(s) Employment history Healthcare affiliation history Malpractice Insurance records
Education Records	This includes information about your education, such as degree, transcripts, or education verification records.
Internet or Website Activity	This may include information regarding an individual’s interaction with our website.

We collect this information from the following categories of sources:

Directly from you when you interact with us, such as when you use our website or related services.
From healthcare organizations that you are applying to for employment or affiliation.
Primary sources, such as your educational institutions, licensing boards, etc.

Use of Personal Information

We use the personal information we have collected from consumers for the following purposes:

Complete credentialing and/or background screening activities.
Comply with legal and regulatory requirements.
Ensure CheckMedic operates correctly.
Improve our services and website functionality.

Sharing Personal Information

Verisys does not sell, trade, or otherwise transfer personal information that you provide to us to third parties, except as indicated below. We do not and will not use any personal information for marketing purposes.

All organizations must certify that they have your authorization to conduct background screening and/or credentialing activities. We will only share your MedPass, and the personal information it contains, with the organization(s) you have authorized. Profiles are not shared across multiple organizations. This means that a new MedPass is created for each organization with which you seek employment or affiliation.

We may disclose your personal information to third parties only for the following purposes:

To comply with legal obligations such as a court order.
To protect against unauthorized access or other misuse of our website or related services.

Your Rights

You have the following rights:

You have the right to know what personal information we collect, use, disclose, and sell about you.
You have the right to request that we delete any personal information we collect about you.
You have the right to opt out of the sale of your personal information.
You have the right not to be discriminated against for exercising any of these rights.
You have the right to request to limit the use and disclosure of sensitive personal information.
You have the right to request correction of inaccurate information.
You have the right to opt-out of automated decision-making technology.
